Occupational Therapist – Grade 2 and 3

About the Role:

As an Occupational Therapist, you will play a vital role in supporting clients to maximise their independence and safety at home. Working closely with clients, families, and carers, you will develop care plans and provide practical solutions that enable them to remain active in their local community.

Key Responsibilities:

  • Conduct home-based assessments and provide recommendations to enhance independence and safety.

  • Develop and implement personalised care plans in collaboration with clients, carers, and families.

  • Work within multiple funding streams, including HACC PYP, CHSP, and potentially NDIS (minimal involvement).

  • Prescribe assistive technology and equipment, including scooters and wheelchairs.

  • Conduct home modification assessments with knowledge of Australian Building Standards.

  • Collaborate with Allied Health Assistants to deliver therapy programs and facilitate equipment trials.

  • Work closely with an interdisciplinary team to provide holistic care.

  • Conduct community-based interventions to promote engagement and mobility.

Additional Responsibilities – Grade 3:

  • Lead service development and quality improvement initiatives to enhance Occupational Therapy services.

  • Provide clinical leadership and expertise within the interdisciplinary team.

  • Supervise Grade 2 staff to ensure safe and effective client care.

  • Contribute to strategic planning and evaluation of service models.

What We’re Looking For:

  • Qualified Occupational Therapist with relevant registration.

  • Strong experience in community-based care, including home visits.

  • Expertise in home modifications, assistive technology, and mobility assessments.

  • Familiarity with funding schemes such as Geat2GO and SWEP.

  • Ability to work independently with minimal supervision.

  • Excellent care planning and assessment skills.

  • A commitment to client-centred care and improving quality of life.

  • Candidates must have their own car
