Talent Quarter are proud to have exclusively partnered with one of South Western Sydney’s leading providers of Aged Care to appoint a Dementia Care Coordinator in their growing Quality Team.

This newly created and unique role presents an exciting opportunity for an experienced Registered Nurse or Behaviour Support Practitioner to take the lead in shaping compassionate, person-centred dementia care across the organisation. As a key member of the Quality team, the Dementia Care Coordinator will bring clinical expertise, compassionate leadership, and strong commitment to delivering evidence-based dementia care to oversee and drive the delivery of dementia care services organisation-wide.

As the Dementia Care Coordinator you will lead with purpose, shaping dementia care that empowers residents, supports families, and inspires staff as a key part of our growing Quality team. This role is a unique opportunity to utilise your clinical expertise, compassionate leadership, and a commitment to delivering person-centred dementia care to support dignity, independence, and quality of life.

Responsibilities:

  • Leading the delivery of dementia care services across the organisation.
  • Developing and implementing individualised, person-centred care plans
  • Supporting and educating staff in dementia care best practices
  • Collaborating with multidisciplinary teams and external specialists
  • Fostering a dementia-friendly environment and culture
  • Engaging families, providing emotional support, and facilitating support groups
  • Driving innovation through research, quality improvement, and policy development

About you:

  1. Behaviour Support Practitioner or Registered Nurse qualification
  2. Minimum 3 years’ experience with supporting dementia or other functional cognitive disorders
  3. Expertise in dementia care principles and behavioural strategies
  4. Key focus on continuous improvement and dementia education
  5. Be focused on providing an evidence based approach to care
  6. Strong communication and leadership skills
  7. Experience in mentoring and training staff
