About the Role:

We are seeking a highly skilled and compassionate Clinical Care Manager for a temporary opportunity. In this key role, you will be responsible for overseeing the day-to-day management of care planning and clinical care for aged care services. The position is designed to ensure the effective and efficient delivery of person-centred care, in compliance with relevant professional standards and legislative requirements.

Key Responsibilities:

• Lead and guide the care management team to ensure the highest quality of care for clients.
• Oversee care planning, clinical care, and service delivery to meet individual needs.
• Ensure all services are delivered in accordance with professional standards, legislative requirements, and best practices.
• Manage the reporting and documentation of care services, ensuring compliance with regulations.
• Promote a culture of continuous improvement, fostering a responsive and compassionate environment for clients and staff.
• Collaborate with multidisciplinary teams to enhance care delivery and client outcomes.

About You:

• A Regsitered Nusre with 5 years of experince especially in aged care.
• A qualification in Nursing, or a related field, with a focus on aged care management.
• Proven experience
• Strong knowledge of relevant legislation, standards, and best practices in aged care.
• Demonstrated ability to lead a team, manage clinical care, and improve care quality.
• Excellent communication, problem-solving, and organizational skills.
• Ability to work effectively in a multidisciplinary team.
