SOCIAL WORKER/CASE MANAGER – AGED CARE

Salary: $45 – $55 per hour + Superannuation

Location: Western Sydney

Contract: 3 – 6 months ongoing and with possible permanency options

Hours: Monday – Friday with 4:40pm finish each day. Full Time or Part Time considered.

We are looking for an experienced Social Worker to work in the position of Case Manager. This is a community aged care position in which you will provide slow stream, goal based and time limited support and therapy to older adults who have been discharged from hospital.

You will provide therapeutic interventions, connecting clients to aged care services within the community, and providing support to carers and working with clients to adjust to their new level of functions, or plan for the future.

ESSENTIAL CRITERIA

• Fully qualified Social Worker with relevant qualification
• AASW eligibility
• Previous experience working with aged care clients
• Strong Social Work experience
• NSW Drivers Licence
• Working With Childrens Check or willingness to obtain one
• National Police Check or willingness to obtain one
• Covid-19 vaccinated
