As an RN in this organisation, you will play a vital role in providing holistic care to vulnerable people experiencing isolation, who have been marginalised from accessing health and welfare services, at risk of homelessness and experiencing chronic-complex health conditions.
You will collaborate with a multidisciplinary team to deliver patient-centred care, promote health education, and ensure the highest standards of clinical practice are maintained. Assesses, plans, implements, and evaluates nursing care in collaboration with individuals and the wider multidisciplinary health care team to achieve goals and health outcomes.
Takes a leadership role in the coordination of nursing and health care enhancing individual’s independence and interdependently, assuming accountability and responsibility for their own actions and delegation of care to enrolled nurses and other health care workers (assistants in nursing/support workers).
Currently registered as a Registered Nurse (Division 1) with the AHPRA
Current NSW Driver’s License
Demonstrated ability to work collaboratively within an interdisciplinary team
