Grade 2 Senior Social Worker for private Hospital located east of Melbourne.
On offer
- Permanent Part-time 32hrs p/w
- Great Location – Easy to travel to
- 8hr Shifts 4 days p/w
- Between $40.07 – $46.07p/h + Super + Salary packaging
This is a unique opportunity to work as a sole clinician in a very team oriented department, working closely with Allied Health Manager and Occupational therapy team. You will identify goals and health outcomes in an acute hospital environment.
As a Grade 2 Social Worker, your responsibilities will include:
- Working independently as part of the health care team
- Adhere to the company’s site policies and procedures
- Demonstrate a commitment to continuous quality improvement, Occupational Health and Safety
- Deliver a high standard of customer service at all times
- The Grade 2 Social Worker may also be required to provide clinical supervision to students
- Clinical leadership and participation in senior forums with the Hospital and Board.
- Undertake comprehensive psycho-social assessments and complex discharge planning
- Engage positively with all other colleagues to achieve best clinical outcomes
- Share your knowledge and practice wisdom with others
Requirements to be successful in obtaining position:
- Appropriate tertiary qualifications in Social Work
- At least two years post-qualifying experience in acute hospital (or related) setting
- Eligibility for membership of the Australian Association of Social Workers (AASW)
- Ability to work autonomously
- Well-developed communication skills and interpersonal skills
- Expertise in aged care/residential services and public referral pathways (ACAS/TCP/GEM) and DHS/VCAT)
- Excellent organisation, time management and self-management skills
- Ability to work within and provide leadership within a multidisciplinary team
- Sound clinical judgement and skills in risk assessment and management
- Comprehensive skills in assessment, discharge planning and counselling
- Commitment to ongoing education and the provision of evidence based practice
- Works very collaboratively with occupational therapist
