Job Title: Care Manager
Start Date: Flexible start date
Location: Western Australia
Salary: 77k – 85k per annum

As a care manager you will be delivering care, support and accommodation for our aging population and those living with mental illness.

Responsibilities

• Coordinate and manage nursing care plans aligned with customer needs and goals, using a structured case management approach
• Collaborate within a multi-disciplinary team, utilising a consultative approach to care
• Build and maintain strong relationships with clients, families, and stakeholders, ensuring top-quality service
• Liaise with preferred providers for assessments, treatments, and service recommendations

Requirements

• Proven experience in coordination or case management within the aged care sector
• Experience working with Indigenous communities
• Current AHPRA registration
• Valid National Police Check
