Adult Case Manager- Community Alcohol and Drug Service

Location: Geraldton

Contract: 3-6 months

Salary: $60-$70 per hour + Super

Travel included with accommodation heavily subsidised

The role:

• Responsible for the assessment, planning, implementation, facilitation, and evaluation of the delivery of patient care
• TO provide mental health care and case management to patients
• Plan, facilitate and coordinate clinical activities and therapeutic interventions for mental health consumers on allocated case load, their carers and family
• Coordinate multidisciplinary services to ensure high standard of health care delivery

The ideal candidate will have:

• Qualifications in social work, occupational therapy, case-management or psychology and registration with relevant professional body
• Demonstrated competence in planning, coordination and decision making, in the management of patients, within a mental health setting
• Experience in community mental health and alcohol and other drug facilities
• National police clearance within last 12 months
• Working with Children Check or willingness to obtain
• Covid-19 Vaccination and relevant immunisation history
